Class of Housing/Services: permanent supportive housing to single adults and families

General Information:
CCIH provides scattered site permanent supportive housing to single adults and families. Apartments are scattered throughout Contra Costa County,  and mobile service teams of case managers,  life skills and youth advocate support staff, and property managers work in partnership with residents to set and achieve goals related to increasing independence and maintaining housing stability. The Scattered Site Supportive Housing Program (SSSHP) launched in June 2015 to provide permanent, affordable housing and support services for 48 formerly chronically homeless adult individual residents living with disabilities. The Families in Supportive Housing (FISH) Program launched in October 2016 to provide permanent, affordable housing and support services for 20 formerly chronically homeless families with special needs. Both programs provide a cost-effective housing resource that prevents use of high cost emergency room visits, hospitalizations, and incarcerations, and supports adults and families to provide a healthy and wholesome environment for themselves and their children. CCIH partners with landlords throughout the county who support our mission by leasing units for CCIH’s residents.
