Class of Housing/Services: Permanent Housing for Chronically Homeless Persons with Disabilities

General Information:
he Gateway program serves people who:     Have been chronically homeless (definition: Living in a shelter, on the streets, or in a vehicle for the past year; living in an abandoned building or in a building like a garage with no utilities or water.     Or     Have been homeless (same definition as above) for 4 separate periods over the past 3 years with the 4 periods totaling 1 year.     And     Have been diagnosed with a 'sever mental illness' and/or substance abuse as diagnosed by a psychiatrist. The Gateway program uses the Housing First model. This means that our clients are housed first and then they are encouraged to address the issues that might have contributed to or caused their homelessness. (This model is considered a best practice.) The majority of new clients come to the Gateway program through referrals from one of the area’s mental health centers or homeless shelters. We work with clients to find the right housing for them within the limits of the funding that is available. Those funds come from the Department of Housing and Urban Development (HUD) and the Indiana Housing and Community Development Authority (IHCDA). Each year we receive funding for:     40 – 1-Bedroom units at the Fair Market Rate (which is currently $646 including utilities) for 12 months     10 – 2-Bedroom units at Fair Market Rate (which is currently $805 including utilities) for 12 months Clients with income are required to pay 30% of it towards their rent and utilities. We must provide a 25% match and a 150% leverage. Thus, government, client and our agency all are invested in this program. Housing can be located anywhere in Lake County. We have clients living in Gary, Hammond, Merrillville, Hobart, Lake Station and Whiting. Landlords must agree to work within the requirements of the Gateway program. All housing must meet HUD-approved standards which are verified by inspection prior to lease-signing. Once the client has signed a lease with the landlord and documents between Catholic Charities and the landlord are signed, the Gateway program pays the security deposit and begins rent payments. The client is supported by the Catholic Charities Case Manager from the beginning of this process, through move-in, and beyond. Once the client has moved-in, the Case Manager visits the client at least twice a month, with one of those visits in the client’s home. While in the program, the client works toward three specific global goals:     Achieving and maintaining permanent housing     Achieving greater self-determination     Increasing income and/or non-cash benefits
