General Information:
Reintegration OUTREACH: Homeless Community Treatment Team provides outreach services to the mentally ill homeless of Palm Beach County and support to individuals who are or have been homeless and are now ready to reside in a community setting. The Team provides supportive services to include, but not limited to, Case Management and Supportive Housing. HOUSING AND RESIDENTIAL: Safe and affordable housing is available for individuals with mental illness, and co-occurring mental illness and substance abuse, as well as individuals with AIDS, where there is a need for additional support and structure. SUPPORTED EMPLOYMENT SERVICES: Persons with severe and persistent mental illness are offered work-readiness training, on-the-job support, advocacy and job-retention techniques for employment and placement. The goal of this program is to assist them in reaching their potential in the job market, with ongoing support for as long as needed.
