General Information:
Whether you are a single veteran or a homeless family, our mission is to reduce homelessness and addiction in Central Florida. We are committed to providing transitional and permanent supportive housing. The Transition House, Inc., offers addiction recovery support services. The Transition House, Inc. has more than 20 years of experience working with the homeless in Central Florida. Our licensed and qualified staff provides case management and therapeutic services dictated by the needs of each of our clients. We also offer substance abuse and addiction programs, in residential and outpatient settings, tailored for each client that seeks help from us. The Transition House, Inc. is a Homeless Veterans Per Diem Grant program offering services that help our homeless veterans mainstream back into society with a sense of personal accomplishment, self-esteem and confidence. Our Permanent Supportive Housing program, Victory Village, is available for qualified families. Targeted Case Management is offered by on-site staff at Victory Village. Through the years, The Transition House, Inc. has developed a wide network of resources for the homeless including educational opportunities, vocational training, addiction counseling, life skills training and whenever possible family reunification and involvement. It is our expert opinion that in order for a person to obtain and sustain permanent housing they must first acquire the necessary skills needed to maintain stability. Fees for services: We accept Medicare, Medicaid, or 30% of adjusted gross income.
